For new counseling clients of Dr. Dave Friese, click here to download his packet of intake forms. If the client is a minor, please click here to download a packet that includes the consent to treat a minor. Please print the forms, fill them out as directed, and bring them with you to your first session.

For new counseling clients of all other Restore & Rebuild therapists, after you have scheduled a session with your therapist, you will receive an email with a link to our secure client portal. You will receive all your intake forms through that portal.
